Role Overview
Medica is seeking a Healthy Communities Program Manager III to lead and operationalize the organization’s Healthy Communities strategy across the enterprise. This role focuses on advancing population health through community-informed, evidence-based initiatives that improve healthcare access, engagement, experience, and outcomes across diverse populations.
The position serves as Medica’s internal subject matter expert on community-driven population improvement and works cross-functionally with teams across the organization to translate strategy into measurable outcomes. The role also partners with external organizations, foundations, and public agencies to strengthen community impact and alignment.
Responsibilities
- Lead and maintain Medica’s Healthy Communities framework using evidence-based and community-informed approaches.
- Serve as the internal subject matter expert on population health improvement, healthcare access, community trends, and emerging policy issues.
- Develop and manage Healthy Communities initiatives from planning through implementation, evaluation, and sustainment.
- Coordinate cross-functional work with teams across Population Health, Quality, Clinical, Product, Finance, HR, Public Affairs, Data & Analytics, and other departments.
- Design scalable initiatives that support Medicaid, Medicare, Commercial, and Exchange populations across multiple markets.
- Establish governance structures, reporting timelines, milestones, and success measures for community initiatives.
- Partner with leadership on strategic prioritization, execution planning, and funding decisions.
- Collaborate with the Medica Foundation, community organizations, and public agencies to align investments and outreach efforts.
- Track program performance, evaluate impact, and communicate outcomes and risks to leadership and stakeholders.
- Support integration of Healthy Communities principles into broader enterprise initiatives and decision-making processes.
Qualifications
- Bachelor’s degree or equivalent experience in a related field required.
- Minimum 5 years of experience in healthcare, public health, population health, community-based work, or a related field.
- Experience leading complex cross-functional initiatives and translating strategy into measurable results preferred.
- Strong collaboration, facilitation, project management, and influence skills across multiple organizational levels.
- Excellent written, verbal, and presentation communication skills.
- Experience working with community organizations, foundations, or public agencies preferred.
- Experience using population-level data to support program development and evaluation preferred.
- Knowledge of healthcare delivery systems, payer operations, or population health models preferred.
- Demonstrated cultural intelligence and ability to work effectively with diverse communities and perspectives.
Payment
Salary range: $78,700 – $134,900 annually.
Typical hiring range: $78,700 – $118,020 depending on experience, education, certifications, and internal equity.

Schedule
Full-time hybrid position.
Employees are expected to work onsite approximately 3 days per week.
